Salary Range: $60k - $63k /Exempt
Founded in 1980, Neighborhood Self Help by Older Persons Project (SHOPP) is a non-profit social service agency located in the Bronx serving older adults and families caring for an ill, frail, or disabled loved one. SHOPP’s mission is based on the philosophy of self-help. We believe that older adults are capable of helping themselves and helping others improve their capacity to deal with the physical, emotional, social, and financial challenges they face as they age.
SHOPP has several service programs that includes Community Care Network Program (CCNP); VIP Elder Justice; NY Connects of the Bronx; Case Management & Friendly Visiting Program; Caregivers Program, Lafayette Estates NORC Program; Older Adult Centers: Casa Boricua, Leon, Soundview, and Guess.
Job Description
The purpose of the Neighborhood SHOPP Case Management Program is aimed at maintaining and assisting homebound older adults in living independently in the community and reducing social
isolation. Utilizing a strengths-based approach, Case Management objectives include identification of needs and capabilities through a comprehensive intake and assessment process, determination of eligibility for DFTA-funded in-home and social adult day services, home-delivered meals, and other supportive services; monitoring, advocacy, linkage to community services and resources, follow-up on referrals and service provision. The Case Manager reports directly to the Social Worker Team Leader.
The position requires a Baccalaureate Degree in Social Work from an accredited school and experience in a social service setting. Knowledge and experience with the older adult population and community based services are also preferred. Essential competencies, knowledge, and skills include advocacy, brokerage, care coordination, communication, problem-solving, collaborative teamwork, and a dedication to empowerment and strength-based social work practice. The ability to work with a diverse population and Bi-lingual skills in Spanish is helpful. Computer literacy is mandatory, and knowledge of STARS is also helpful. The Case Management program follows a hybrid schedule with mandatory monthly home visits in the South Bronx. Conducting home visits may require walking up and down stairs. All staff must be able to travel and report to the office 2-3x a week and attend in-person training, meetings, or events.
